Description of the Webinar

Adolescents and young adults (AYA) with pediatric-onset chronic kidney disease (CKD) have great survival and will transfer to adult-focused health care. These AYA need to undergo health care transition preparation. While the process of health care transition begins in the pediatric settings, adult-focused providers need to continue to assist these AYA to successfully self-manage their disease and co-morbidities. In this webinar we will discuss how to prepare AYA to self-manage their conditions, based on measurement tools and lessons from the field.

About the Facilitator

Ferris2011 

Dr. Maria Ferris is an Associate Professor of Pediatrics and Medicine at the University of North Carolina at Chapel Hill, Dr. Ferris has founded the UNC Health Care Transition Program. She is a pediatric nephrologist and epidemiologist who also directs the Pediatric Renal Replacement Therapy Programs, while also manages to have a family. One of her 3 children is a young adult with special health care needs. Her mission is to develop evidence-based health provider and patient tools to improve the outcomes of young adults with pediatric onset chronic conditions.
