Below you will find important information and various forms regarding the application proccess. The Fellowship program has two application sessions per year – December 15 and June 15. It is mandatory to submit all the documents in sequence order in PDF format. Fellows who are already in training and wish to apply for an extension must submit the Fellowship Extension Application form. Please note that fellows who have already completed their training are required to sumbit an End-of-Training Report.

** End-of-Training Reports must be reviewed and approved by the Chairman before the above-mentioned advantages may be granted. The report must be submitted within three months following the completion of the Fellowship training. If submitted later, the Fellow loses his/her privileges of membership and guaranteed WCN Travel Grant.
