What is the differential diagnosis of persistent itching+ papular skin rash in a diabetic HCV +ve hemodialysis patient?

the condition persisted for more than 6 months inspite symptomatic treatments, severe, the rash is all over the body, small papules less than 5mm, extremeties, trunk and back. sometimes burning eye sensation. pinkish, not like Lichen Planus or scabies. the patient is 45 year old male, healthy looking, controlled Ca, P and PTH levels.normal complete blood counts. the itching does not improve with dialysis or changing the treating dialysis center. he has a back pain.
